Susan Troesch, Diabetes Educator

Clinical Pharmacist, Mid-Main Health Centre

Troesch is a community pharmacist, clinical instructor for the Faculty of Pharmaceutical Sciences at UBC and Certified Diabetes Educator (CDE) working at Mid Main Community Health Centre and some VCH primary care practices in a clinical role beginning in 1999. Involved in the facilitation and delivery of care with primary care physicians using the Group Appointment model for a number of populations since the early 2000s.

Carolyn Hoeschen, Nurse Practitioner

Nurse Practitioner, Mid-Main Community Health Centre

Hoeschen is a nurse practitioner at Mid-Main Community Health Centre for over one year, working in shared practice care model with family physicians at the center. She joined the co-facilitation of group medical visits along with the Clinical Pharmacist, with focus on chronic disease management, health promotion and patient education.

West Coast Family Clinic (Sooke)

Dr. Ellen Anderson, MD

Clinical Assistant Professor

Dr Anderson has been a full service family practitioner in BC since 1980. She currently practices in Sooke BC at West Coast Family Medical Clinic, and enjoys working as part of an integrated health network , which includes 7 family doctors, a chronic disease management nurse, dietitian and social worker, and includes shared care with visiting specialists for both child and youth


Elizabeth (Beth) Walsh, Nurse Lead, IHN

Registered Nurse, Sooke Integrated Health Network

Walsh began her registered nursing career in the mid 1980's, initially working in the Peace River region of the Province as a general duty nurse providing outreach home nursing care to the rural communities. Nursing continued in Prince Rupert and to multidisciplinary work in the area of cardiac and diabetes management.

Darcy Patterson, Physiotherapist

Physiotherapist, Vancouver Island Health Authority

Patterson has been practicing physiotherapy twenty years, primarily in public health, with some private work in the areas of vestibular rehabilitation and as a certified stretch teacher. She achieved her BSc. Kinesiology (formerly known as Human Performance) from UVIC 1990, and then went on to complete her BSc in PT from the U of Saskatchewan 1994. With a core value
